Who will be my care team?
The entire team at The Christ Hospital Center for Health and Aging is specialized in caring for the complex needs of aging adults. The team proactively anticipates complications that will arise. Often times, this is done through monthly face-to-face meetings and frequent telephone contact with you or your caregiver. Ongoing communication helps monitor your health-related signs or symptoms and provide your team with an opportunity to make quick decisions for early changes through relatively simple outpatient interventions.
