The ACE Unit at The Christ Hospital is a 10-bed unit located on 4 South at our Mt. Auburn campus. The unit is the only one of its kind in Greater Cincinnati. It is staffed by a highly specialized geriatrics care team who works alongside the treating hospital physicians to survey and assess for the special needs of older patients and their loved ones in order to achieve the outcomes patients and families expect. The ACE’s purpose is to work diligently to prevent physical and mental decline of older adult patients whose condition warrants hospitalization.
Our geriatric care team includes a geriatrician, a geriatric nurse practitioner, social worker/case manager, registered nurse, registered dietitian, pharmacist, pastoral care representative and physical and occupational therapists. Your care–team will visit you and your loved ones daily during rounds throughout your stay.
Daily rounds are an opportune time to give and receive updates from your care-team and the best way for everyone to discuss your progress and treatment plan. During daily rounds your nurse, doctors, aides, and therapists update you and your family about your diagnosis, testing, treatments and progress. We will also address pain or any other symptoms that you may be having. Part of our daily rounding also includes planning for discharge with our social worker to discuss where, when and how your needs can be met after your hospital stay.
The Mobile ACE Unit is also staffed by a highly specialized geriatrics care team who works alongside physicians, patients and their loved ones as well as other caregivers to prevent physical and mental decline of older adult patients whose condition warrants hospitalization. The same additional support and care in the ACE unit can also be provided throughout the hospital to patients admitted to our geriatric hospital service who are followed by the Mobile ACE team.
The Mobile ACE Unit team specializes in patients that are high-risk or fragile throughout the hospital. The team is staffed by geriatricians who will work alongside physicians, patients and their loved ones as well as other caregivers to prevent physical and mental decline of older adult patients whose condition warrants hospitalization.
For the Mobile ACE team, we make available regular updates to you and your support system from your care-team. Family meetings are offered in order to discuss your progress, treatment plan and expectations of those plans with you and your loved ones. An additional focus of Mobile ACE is to include you and your family in planning for discharge. Our social worker is there to discuss your expectations as to where, when and how your needs can be met after your hospital stay.
We are the regions’ only hospitalists’ service, staffed by geriatricians, internists and family medicine specialists with a primary focus is on the special needs of older adults. We complement our Internal Medicine and Family Medicine hospitalist services. Geriatric hospitalists often serve as the admitting physician for the more vulnerable, older adults requiring a hospital stay. They are specially trained to treat older patients, manage multiple disease symptoms, develop care plans that address the special healthcare needs of older adults and support those caring for the aging adult.
