​​​​​Diabetes self-management education is designed to provide up-to-date information, self-care tools and resources in order to set goals and modify current behaviors to promote glucose control.


Our comprehensive program begins with an initial one-hour assessment to determine your educational needs. The length and type of education is individualized to help you and your designated diabetes support person understand your diabetes management.

 

Individual and group training is provided by certified diabetes educators (nurses and dietitians) for adults diagnosed with:

  • Type 1 diabetes

  • Type 2 diabetes

  • Gestational diabetes

You and your diabetes support person can learn about:

  • Being active

  • Healthy coping

  • Healthy eating

  • Monitoring

  • Problem solving

  • Reducing risks

  • Taking medication

Ongoing support is provided to help with any issues or concerns that arise.

 

Our certified diabetes educators also provide specialized training in:

  • Data management

  • Delivery devices (pumps)

  • Glucose monitoring devices (CGM)

  • Intensive insulin therapy

Appointments

Appointments can be scheduled by you or your physician’s office for any location. A written referral for diabetes self-management training from your physician is required. Services are offered Monday through Friday. Evening hours are available on certain days and at certain locations. Contact 513-585-1218 for more information.

 

Insurance Coverage

Diabetes self-management education is a covered benefit on many health insurance plans. Please confirm your health insurance benefits prior to your first appointment. Here are the steps to help you verify your insurance coverage:

  • Contact the “800” number on the back of your insurance card

  • Use the term "diabetes self-management training" to describe our service

  • Identify the location where you will receive education (hospital or physician office)

  • Ask how many hours and/or number of visits covered.
