The senior care experts at The Christ Hospital Health and Aging Center provide coordinated, comprehensive care for older adults with symptoms of memory loss or other cognitive impairment.

These include many forms of dementia and disorders linked to dementia due to:

  • Lewy body dementia disease

  • Vascular dementia disease

  • Mixed dementia

Who the Health and Aging Center serves

When you or someone you care about start showing signs of cognitive decline, professional care can help. You could benefit from our services if you or a loved one is experiencing:

  • Memory loss and disorientation

  • Changes in mood and interests

  • Irrational reasoning and impaired comprehension

  • Loss of vocabulary

  • Balance problems

  • Decline in self-care and hygiene

Services at the Health and Aging Center

We see beyond our patients' cognitive decline and focus on improving their overall health and quality of life.

We offer:

  • Access to board-certified geriatric specialists with a wide range of expertise

  • Cognitive assessments and depression screenings

  • Education and resources for caregivers regarding safety concerns, including driving and living independently.

What to expect during your initial visits

When you become a patient at the Health and Aging Center, your entire support system gains access to a level of care seldom experienced anywhere else. We make time in each of our appointments to get to know you and your caregivers.

We value the input of the people who know you best, and we use their insights to help craft your care plan.

Your first two appointments will be longer than typical doctor visits. These appointments are specifically designed to develop a comprehensive care plan. Most of our time together will be spent in relaxed discussion.

At your first visit

During the first appointment, you will have an hour and a half to meet with a Certified Nurse Practitioner or Physician Assistant for a detailed review of your medical and personal history, and a brief physical examination. We strongly suggest a close caregiver accompany the patient for this visit.  

At your second visit

The second appointment gives you up to three hours to meet with one of our physicians and social workers for a comprehensive assessment. You will have time to discuss your goals and concerns, and to receive guidance about your personalized care plan.

Just as important, you'll gain confidence that you and your family will get the right care and support! We strongly suggest your closest caregivers come for this detailed appointment.
