Immigration physicals at The Christ Hospital Urgent Care

The United States Citizenship and Immigration Services (USCIS) requires green card and U.S. citizenship applicants to receive a full physical. USCIS designates certain doctors (also known as civil surgeons) to perform the required medical exams.

 

Civil surgeons Robert Hollis, MD, and Alison Sweet, MD, with The Christ Hospital Urgent Care in Fort Wright, Kentucky, are now offering immigration physicals by appointment.

What to bring to the exam

  • Government-issued photo identification, such as a valid passport or driver's license. Patients ages 14 and younger will need identification that shows their name, date and place of birth, and parent's full name. Possible forms of identification include birth certificate (with an English translation) or an affidavit.

  • Vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease)

  • Form I-693, Report of Medical Examination and Vaccination Record, available from the USCIS website

  • Payment (see below for details)

During the exam

During the exam, the doctor will:

  • Perform a physical exam including blood pressure and other vital signs, height and weight

  • Test for tuberculosis (TB) (Note that your TB test will be done in one step, so you will not need to return to the office to have it read. However, if your TB test is positive, you will need to return to the office for a chest X-ray.)

  • Test for syphilis and gonorrhea (patients 15 and older)  

  • Review the vaccination records you bring with you to see if any additional vaccinations or testing is needed. If additional vaccinations are needed, you can get them through your primary care office – and they can be billed to your insurance plan.

  • After your blood and urine test results are sent back to us from our lab (usually within seven business days), the doctor will complete Form I-693 and seal the form in an envelope for you to pick up and submit to USCIS.

Cost of exams

The cost of your exam, including blood and urine testing, and completion of required documents, is: 

  • $350 for patients 15 and older 

  • $200 for patients 18 months through 14 years of age

Please note that:

  • Additional charges may apply if you need further testing (for example, $97.20 for a chest X-ray and professional read fees).

  • The entire amount is due prior to services being rendered.
