Please answer a few questions to confirm eligibility for your first dose (new questions will display after you answer each question).
If you meet the requirements, you'll be prompted to schedule your appointment. The process takes about five minutes. If no appointments are found, try again later; we expect to add appointments weekly.
The Pfizer vaccine is available for everyone 16+ beginning March 29. Minors will need to be accompanied by a parent or legal guardian who can sign the vaccine consent form and be available in the event of side effects (although extremely rare). If for some reason we don't receive the Pfizer vaccine on a day minors are scheduled, we'll reach out to anyone affected to reschedule for another day.
While your Social Security number (SSN) and insurance information aren't required at the time of scheduling, they're highly recommended in order to streamline registration the day of your vaccine. (Please don't enter dashes in your policy number.)
If you suffer from severe allergic reactions that require you to carry an EpiPen, or if you've had an allergic reaction to other vaccines or injectable medications, please speak with your provider prior to scheduling the vaccine (allergic reactions have been reported with the mRNA vaccines from Pfizer and Moderna – this may be the result of an ingredient that is also found in some medications including those used to treat cancer).
