COVID-19 Vaccine Scheduling Options 

We now offer several convenient ways to schedule your first COVID-19 vaccine at our main campus in Mt. Auburn and at 20 primary care offices located throughout the Tristate. 

OPTION 1:  

Schedule your appointment through your existing MyChart account. 

If you have MyChart, log in to your account (through the web or app), click Visits | Schedule Appointment | Tell us why you’re coming in | then COVID-19 Vaccine #1. From there, answer a few questions about your health, pick a date, time and location, then Schedule. A few important tips

  • You won't see the vaccine scheduling button unless you meet the eligibility criteria for the vaccine. 

  • Be sure to click the navy blue "Schedule" button at the end of the vaccine scheduling form, otherwise, your appointment will not be scheduled. 

  • If scheduling your appointment through MyChart, please log in to your account several days before your appointment to complete registration (eCheck-in). By checking in ahead of time, your appointment will go faster the day of your visit. Just click on the dark blue "eCheck-in" button near your appointment details. 

OPTION 2:  

Sign up for MyChart to schedule your appointment.

If you've had an appointment at The Christ Hospital Health Network previously but do not have a MyChart account, you can sign up for one here by answering a few short questions. When you meet the eligibility guidelines for the vaccine, just follow the steps above to schedule. 

OPTION 3:  

Schedule your appointment through our website. 

If you don't have a MyChart account, do not wish to sign up for one, or have never been a patient at The Christ Hospital Health Network, click to schedule your appointment here through our website. A few important tips:

  • This option is available for our Joint & Spine Center location only. If you'd like to schedule at one of our participating primary care offices, please use MyChart or call the provider's office directly.   

  • If you have insurance, be sure to have your insurance card handy. While we don't require you to enter your policy information at the time of scheduling, we highly recommended it so that we can streamline registration the day of your visit. (Please don't enter dashes in your policy number.)
  • While your Social Security number (SSN) isn't required at the time of scheduling, it's highly recommended in order to streamline registration the day of your vaccine. (We ask for SSN in order to verify your insurance - and identity for patient safety purposes. Information is stored on a secure server and not shared/sold.)

  • Be sure to click the navy blue "Schedule" button at the end of the vaccine scheduling form, otherwise, your appointment will not be scheduled. 

  • Upon arrival to the vaccine clinic, you will be asked to show a photo ID to verify your age. 

OPTION 4:

Schedule your appointment by phone. 

  • To schedule your appointment at our main campus in Mt. Auburn, call 513-585-3881Phone lines are open Fridays, 8 a.m. - noon. 

  • To schedule your appointment at one of our participating primary care offices, please call the office directly. 

When scheduling by phone, please have your insurance card, date of birth and social security number ready.

OPTION 5:

Schedule your appointment through the ODH Website (Apothecary location only). 

We expect a limited number of appointments to be available each week in The Christ Hospital Apothecary (the pharmacy located inside Joseph Beth Booksellers near our main lobby). Scheduling for this location is available through the Ohio Department of Health's (ODH) Website. Just search Zip Code 45219 then choose The Christ Hospital Apothecary to locate appointments.
