Leading the way in heart failure management
The Christ Hospital Advanced Heart Failure Program has been a national leader in heart failure care for the past decade. The Christ Hospital Heart Failure Network is an extension of this renown program, and one of the first of its kind in the U.S. The network is dedicated to improving care for advanced heart failure patients in our community by making the most advanced care and treatments accessible at partner facilities throughout Greater Cincinnati.
Nearly one in four heart failure patients nationwide are re-hospitalized within 30 days of a previous hospitalization. Two thirds of these readmissions are preventable. To help curb this epidemic, we’re training other healthcare partner providers about our best practices for heart failure care.
Through the Heart Failure Network, our team partners with local healthcare providers to shares best practices in heart failure care. We guide each facility in developing a multidisciplinary team to provide evidence-based practice recommendations. Heart Failure Network partners undergo quality improvement initiatives under the guidance of our experts to improve their heart failure care and reduce hospital readmission rates.
