You can dedicate a donation to someone—a patient, a loved one, a caregiver, or a doctor, in honor or memory of them.

If you’re donating online, simply indicate the person in the Tribute section of the form. 

If you’re donating another way, let the person you’re speaking with know whom you’d like to honor with your donation.
