We value every patient and work hard to make sure you have a great patient experience. The information contained in your medical record is confidential. We cannot release any medical information without patient’s consent or, in the case of a minor, their legal guardian’s consent, unless required by law.​​ 

​Please note that State and Federal laws permit fees to be charged for copies of medical records that are not for continuation of care. ​​

Electronic Request

We offer an online patient record request tool for the following individuals to request patient records:​ 

• Patients requesting their own records 
• Patient requesting their own records be sent to someone else 
• Parents of minor patients requesting records 
• Caregivers acting on behalf of a patient (i.e., Power of Attorney) 

The online tool verifies your identity by asking for a photo of your driver’s license or patient’s government-issued identification, which can be taken via webcam or smartphone. There is no additional charge to use this service.

To obtain a copy of your medical record, print and complete our authorization form. Please make sure to provide your full name, date of birth, the specific records needed and how you want the records ​to be delivered to you. In order to receive your medical records electronically, please provide your current email address.
