Maximize your health and wellness with convenient access to exceptional, personalized care from our team of family medicine and psychiatry residents who are working toward board certification. Our residents are supported by a team faculty of board-certified family medicine physicians and other dedicated care providers.

Our convenient location in the Medical Office Building on the campus of The Christ Hospital in Mt. Auburn gives you direct access to comprehensive medical services including laboratory testing, imaging, physical therapy, and a wide range of subspecialities.

Coordinated Care Under One Roof

Our entire staff strives to provide you with the best care. In addition to your Primary Care Practitioner, staff members you will likely interact with at our practice include:

Schedulers: Who answer phones, take messages, schedule appointments, and manage check-in and check-out
Nurse Midwives: Who provide an individualized approach to delivering a full range of obstetrics & gynecologic care for women of every age
Pharmacists: Who work with you and your provider to manage your medications
Medical Assistants: Who gather information, take vital signs, perform and order tests, and respond to messages
Advanced Practice Providers: Nurse Practitioners and Physician Assistants who provide direct care to patients and expand access to care for acute needs
Behavioral Health Consultants: Who assist patients with a variety of issues such as depression, anxiety, stress, chronic disease management and weight loss
Care Coordinators: Who focus on preventative care and wellness promotion
Care Manager: A registered nurse who works with you on management of complex medical needs
Practice Manager: Who assists with questions about office policies and operations
Resource Social Worker: Who can connect you and your family with community resources
Lab Services: Phlebotomists who draw blood with a physician’s order. Unless otherwise directed for insurance purposes, all blood draws will be analyzed by a Christ Hospital lab.

Your Office Visit and Copays

Please arrive 15 minutes early. Arriving late may necessitate rescheduling.
Please call more than 24 hours in advance to cancel appointments. If you cancel within 24 hours of an appointment, you may be charged a $50 fee,.and if you cancel multiple times, it may result in dismissal.
Copays are collected at the time of your visit.
If you have a high-deductible plan or don’t have insurance, please pay your responsible portion, or a minimum of $50 at the time of your visit.
If you fail to pay an outstanding balance, your appointment me be rescheduled.
Call 877-651-4343 for any billing questions.
Please bring these items to every visit:
- Insurance card
- Photo ID
- List of your medications

After-Hours Care

In the event of an emergency, call 911.

A practitioner is on call after hours for urgent medical issues.

Leave a detailed message
Remember to keep your phone nearby
Have the patient available to speak with the provider, if possible
Be prepared to provide the number of the closest 24 hour pharmacy
Do not call after hours for medication refills, appointments, or test results

Depending on the time and urgency of your need, the on-call provider may instruct you to visit one of our Urgent Care or Emergency Department locations.

Patient Information & Resources

Forms, Letters & Paperwork

Please leave any forms needing to be signed by your provider at the front desk. You will be notified if an appointment is required for completion. Some fees may be applicable.

Patient Forms

MyChart

Access MyChart from your phone, tablet, or computer to manage your care. Create an account online or our staff will assist you.

With MyChart you can:

View and manage appointments
Request prescription refills
See test results and medical records
Send non-urgent messages to your care team*

Parents can access accounts for children under 18 and adult patients can grant proxy access to allow family members and caregivers.

*Please allow two business days for responses to MyChart messages.

Lab & Test Results

Please allow two to three days for most results, and 3-4 days for radiology results. We will notify you by phone, email or MyChart regarding your results. Please call the office if you have not heard from us within a week. You will be able to view your results directly in MyChart when they are available.

Medication & Refills

If you are on prescription medications, you must be seen at least annually
Some medications require more frequent appointments or lab tests. Please schedule required follow-ups in advance of when the refill will be needed
If no visit is required, you can request refills by calling your pharmacy or through our office by using MyChart
Refill requests are not processed on Fridays or after hours
Your provider has the right to deny early refill requests

Referrals, Pre-Certification and Prior Authorization

Before seeing a specialist, please contact your insurance plan to find out if the specialist is in your network. If prior authorization is required, contact us with the following information:

Patient name, date of birth and phone number
Name of specialist or facility
Appointment date and reason for appointment
Name of insurance company
Please allow 72 business hours for referral authorizations.
Once the authorization is received, the referral authorization will be sent to the specialist

Some tests and medications prescribed by our team may also require prior authorization, which we make every effort to obtain in a timely manner. However, it is your responsibility to confirm with your insurance company before obtaining tests and prescription medications.
